The patient, parent, or legal guardian must sign a Release of Information form that authorizes White Rock Medical Center to release copies of his/her records to whom he/she has authorized. You will need to provide a photo ID.

Note: You could be charged more than the estimate if you get additional items or services during your visit or procedure that your doctor didn’t anticipate.

Paying for Services

We accept Medicare, Medicaid and many major health insurance plans, including the HMOs, PPOs, and Managed Care plans. If you have questions or do not have insurance, please contact our Business Office to discuss financial arrangements. In addition, the Patient Access Department can help you with applying for Medicaid or other government assistance programs.
